WELCOME TO LNL SECURITY SERVICES

Step 1: Print and Complete the Security Guard Application

• Print both pages of the Security Guard Application. 
• Complete Page 2 by signing and dating it. 

Step 2: Print and Complete the DMV Consent Form

• Print the DMV Consent Form. 
• Check the Security Guard box. 
• Print your first name, last name, and middle initial. 
• Sign and date the form. 

Step 3: Include Your Training Certificates

• Attach a copy of your 8-Hour Pre-Assignment Training Certificate. 
• Attach a copy of your 16-Hour On-the-Job Training (OJT) Certificate. 

Step 4: Include Your Fingerprint Receipt

• Attach one (1) copy of your fingerprint receipt. 

Step 5: Obtain a Money Order

• Purchase a $36.00 money order. 
• Make it payable to "NYS Department of State." 

Step 6: Review Your Application Packet
Before mailing, ensure your packet contains:
✔ Completed Security Guard Application (signed and dated)
✔ Completed DMV Consent Form (signed and dated)
✔ 8-Hour Pre-Assignment Certificate
✔ 16-Hour OJT Certificate
✔ Fingerprint Receipt (1 copy)
✔ $36.00 Money Order payable to NYS Department of State

Step 7: Mail Your Application Packet
